At Ferncliff, we want to do everything we can to ease the hospital discharge process, so please know that we welcome suggestions from Discharge Planners.

Although we encourage families to tour Ferncliff before they make a decision, we also understand that a visit is not always possible.

We require the following standard information prior to a bed offer by Ferncliff:

• Patient Face Sheet
• PRI
• Medication Record
• Chest X-Ray
• Level 1 Screen
• Level 2 Screen for psychiatric or MRDD referrals (only for long-term residents)

The following are extremely helpful but, in most cases, not required prior to a bed offer:

• Admitting History & Physical
• Psychiatric Evaluation (when indicated)
• Consults
• Blood Work
• Blood & Urine Cultures
